This form is for student's currently attending a Providence Public School.   The form must be accompanied by a valid proof of residency such as a bank statement, pay stub or utility bill dated within the last 60 days, or lease/rental agreement.  Parents/guardians are reminded that all forms must be signed and completed by the parent/guardian of record and the proof of residency must be in their name.   If you wish to submit this form via fax (401-278-0553), please be sure to enclose a copy of a valid photo id.

This form is a request to be placed on a wating list for a preferred school.  It must be completed and signed by the parent/guardian of record.

Parent/Guardian:

This form should be submitted to your school principal for processing.   It should be completed and accompanied by any written documentation confirming the safety issue.   Once you have met with the school principal and the form has been approved, it must be forwarded to School Operation & Support by fax to 454-8935 for approval.  This form is not processed by the center.

Parents/Guardians:

This form should only be used if you currently do not have a proof of residency in the your name as listed on the registration requirements.  NOTE:  In order for this form to be valid, it must be signed, and sealed by a RI Notary Public.
